		<description>I contracted samonella food poisoning from field green lettuce at a salad bar in Macy&#039;s Department Store (State Street in Downtown Chicago) in their Market Place restaurant on Thursday, November 19, 2009. I stayed home from work one day.

For the past 5 days, I have experienced: foul-smelling diarrhea; inflammation and swelling of my intestines; abdominal cramps; weakness; nausea; fatigue; dizziness; headaches; chills; muscle pain; swollen, bleeding hemrrhoids, and weight loss.

How do I prove to Macy&#039;s that this happened and should I contact the Health Department?  Thank you.</description>
